The Reason for an ID card

The ID Card is designed to help people who have a condition known as ADHD/ADD and its co-morbid disorders. Our aims are to help agencies that may come across this condition. i.e.: Schools, School Trips, Social Settings, Police, and Holidays - the list goes on. This explains that the carrier has a Neurological and Medical condition, and is on medication. The information on the card will clearly explain why this person is Authorised to carry a controlled medication.

Should you require a ID card, Complete this online form, and click the “Send Request” button

Full Name Email Date of Birth Address Telephone

We will then send you a form to take to your Doctor along with a passport sized photo of your child or yourself

Ask the Doctor to fill in Part B of this form and sign the back of the photo.

Send the completed form and photo to Our Office.

There will be a £3 administration charge for each card produced. The information will be verified and the card prepared and returned to you in 7 to 14 days.

Please note that any replacement cards will cost £3.
